As I've reviewed my accident, one realization is clear: If a checkbox addressing the flight review had been on our tailboard form, it would have crushed my assumption of its completion. When roles and tasks are well-defined, assumptions cannot take root. Here's how linemen can cultivate clarity to ensure assumption-free operations:

1. Mapping Tasks: Simplifying Complexity into Achievable Steps: Complex tasks can become breeding grounds for assumptions due to their multifaceted nature. Combat this by breaking down these tasks into smaller, specific steps. Assign these steps to the most appropriate team member based on their skills and expertise. This approach eliminates ambiguity and ensures everyone knows their responsibilities within the larger task. By creating a roadmap of sequential steps, linemen can collectively work toward completing projects without room for assumptions.

2. Clear Responsibilities Documented for Reference: Documenting each team member's responsibilities in writing provides a tangible reference point for everyone involved, clarifying tasks and streamlining accountability. A great time to do this is during the pre-job brief. When roles are clearly defined and documented, linemen can reference the documentation as needed, leaving no room for assumptions about who is responsible for what. This practice promotes a sense of ownership and empowers team members to perform their tasks confidently and efficiently.

3. Cultivating Clear Roles - Workshop Approach: Regular role workshops are a proactive way to ensure everyone is on the same page regarding roles, expectations, and team dynamics. Engage in discussions where team members openly share their understanding of their roles and responsibilities. Collaboratively establish guidelines for effective teamwork, communication, and cooperation. These workshops provide an opportunity to clarify any uncertainties, preventing assumptions from arising due to differing interpretations of roles. A shared understanding fosters a cohesive and well-coordinated team.

4. Checklist Culture: A Comprehensive Approach: One of my most used tools, developing task-specific checklists, is a tangible strategy to prevent assumptions. These checklists serve as a detailed guide for linemen to follow. Before progressing with a task, ensure that everyone has completed and ticked off each item on the checklist. This approach leaves no room for assumptions about whether a critical step was missed or misunderstood. Checklists instill a disciplined approach to tasks and serve as a visual reminder of the tasks at hand, contributing to safer and more efficient linework operations.
