Tactical Tuesday Spotlight: Preventing Hazards Through Diligent Inspection
Mar 12, 2024Welcome to another Tactical Tuesday, where we share valuable lessons from the field to reinforce our commitment to safety and vigilance. Today, we delve into an incident that exemplifies the critical role of thorough inspections in preventing dangerous situations.
The Discovery
A seasoned crew, tasked with replacing a rear easement pole with the assistance of a crane, followed their instinct for a routine but vital safety procedure: the hammer test. This simple yet effective test revealed the pole's precarious condition. Without hesitation, the crew implemented temporary supports, using guy wires in multiple directions. They then secured the pole with a crane for additional support—a decision that proved to be crucial.
Upon removal, the crew discovered extensive deterioration of the pole, which had been standing since 1959. The decay ran deep, extending 18 inches below the concrete surface, indicating the pole had been compromised for quite some time. It was a stark realization that the concrete surface was the only thing preserving its upright position.
Proactive Safety Actions
This incident reinforces the necessity of preemptive safety actions:
-
Thorough Visual Inspection: Always perform a visual inspection for any signs of wear, environmental damage, or other indicators that could compromise the pole's integrity.
-
Hammer and Bore Testing: Utilize hammer tests to detect internal decay and bore tests when necessary, especially for poles with an extensive service life.
-
Support Questionable Poles: Proactively support any pole that raises concerns during initial testing before any weight or strain is applied during the job.
Lessons Learned
From this "Good Catch," we take away a fundamental safety lesson: the importance of trusting our training and instincts. The crew's decision to perform additional tests and support the pole likely prevented injury or worse. It also serves as a reminder that what appears stable on the surface can be misleading and potentially dangerous.
Moving Forward
Let this incident be a reminder of the importance of preemptive checks and the power of a proactive approach. Ensuring the safety of our crew and infrastructure requires constant vigilance and a refusal to take shortcuts, no matter how routine the task may seem.
Join us next Tuesday as we continue to share insights and stories that contribute to our legacy of safety, vigilance, and excellence. Remember, safety is a collective effort—every measure we take is a step toward safeguarding lives.
